Section 5-C:82 - Application for and Preparation of a Certified Copy of a Death CertificateI. Written application for a certified copy of a death record shall be made by mail or in person at the division or at any clerk of a town or city's office.II. The applicant for a certified copy of death record shall provide: the full name of the deceased; the date and place of death; the purpose for which the record is requested; the applicant's signature; the applicant's relationship to the deceased; and the applicant's name, address, and telephone number.III. If the registrar determines that the applicant has a direct and tangible interest in the record, the registrar shall issue a complete certified copy of a death record for any year of occurrence on safety paper, bearing the official seal of the state registrar.IV. The local or state registrar shall issue a certified copy of a vital record if the application is incomplete but the information provided enables the record to be identified and located and there is no question concerning the direct and tangible interest of the applicant. 2005, 268:1, eff.
