Section 695D.140 - Certificate of authority: Notice of change of information; application for amendment; fee; approval1. Except as otherwise provided in subsection 2, every organization issued a certificate of authority by the Commissioner shall notify the Commissioner of any change in the information provided to obtain its certificate of authority within 10 days after the change.2. Every such organization that wishes to make a change in the geographic areas which it serves, or to make any other material modification of the operations described in the information required by 695D.120, shall file with the Commissioner an application for the amendment of the certificate of authority and pay to the Commissioner an application fee of $100.
