Section 630.165 - Application and affidavit for license or license by endorsement to practice medicine; additional requirements; burden of proof1. Except as otherwise provided in subsection 2, an applicant for a license to practice medicine must submit to the Board, on a form provided by the Board, an application in writing, accompanied by an affidavit stating that:(a) The applicant is the person named in the proof of graduation and that it was obtained without fraud or misrepresentation or any mistake of which the applicant is aware; and(b) The information contained in the application and any accompanying material is complete and correct.2. An applicant for a license by endorsement to practice medicine pursuant to NRS 630.1605, 630.1606 or 630.1607 must submit to the Board, on a form provided by the Board, an application in writing, accompanied by an affidavit stating that: (a) The applicant is the person named in the license to practice medicine issued by the District of Columbia or any state or territory of the United States and that the license was obtained without fraud or misrepresentation or any mistake of which the applicant is aware; and(b) The information contained in the application and any accompanying material is complete and correct.3. An application submitted pursuant to subsection 1 or 2 must include all information required to complete the application.4. In addition to the other requirements for licensure, the Board may require such further evidence of the mental, physical, medical or other qualifications of the applicant as it considers necessary.5.
