Any insurer, healthcare plan, healthcare professional, or medical-hospital healthcare service provider or any person or entity that fails to comply with any of its responsibilities or obligations imposed by this Act, shall be guilty of a civil penalty and punished by a fine of not less than five hundred dollars ($500) nor more than ten thousand dollars ($10,000) for each incident or violation of the law.”
