[Every person who aspires to obtain a paramedic or basic emergency medical technicians license, shall comply with the following requirements:]
(a) Complete the application for examination on the form provided by the Board.
(b) Recent 2′′ by 2′′ photograph which accredits his/her identity.
(c) Be over 18 years of age.
(d) Have passed an emergency medical technician, paramedic, or basic course offered by an academic institution accredited by the General Education Council or the Council on Higher Education and based on the national curriculum established by the U.S. Department of Transportation (DOT), or by an institution recognized by the corresponding accrediting body with authority therefor and accepted by the Department if such institution is located in any of the states of the United States of America, the District of Columbia, or another foreign country. In addition, he/she must pass a technical course in sign language offered by an institution accredited by the General Education Council or the Council on Higher Education and accepted by the Department of Health.
(e) Present a Criminal Record Certificate issued by the Puerto Rico Police, within six (6) months prior to the date that the application for examination is filed.
(f) Medical certificate.
(g) High school graduation diploma or certificate.
(h) Original birth certificate.
(i) Transcript of credits [passed] by the applicant certified by the competent authority of the corresponding university or college addressed directly to the Board.
(j) Copy of the diploma accrediting the academic degree obtained for his/her studies of emergency medical technology and an official certification of the university or college from which he/she obtained said degree.
(k) Include a certified check or money order payable to the order of the Secretary of the Treasury in the amount specified in these regulations.
In the event the application is denied, the applicant shall be notified of the reasons therefor.
Upon accepting the application, the applicant shall be sent a schedule for admission to the examination indicating the hour, date and place that he/shall appear to take the examination. The applicant shall hand said schedule to the examiner or the representative of the Board on the day of the examination.
