Every public agency or instrumentality shall have evaluation forms available in those offices that provide services to customers, so that they may evaluate the quality of the services received and make suggestions as to changes that may be implemented to improve said services.
Said evaluation forms shall be placed in a drop-box duly identified with the name of the program and the word “suggestions”. Such drop-box shall be placed in a conspicuous place, so that it may be seen by all the persons who visit such public facilities to obtain services.
It is hereby provided that the recommendations made by the customers shall be evaluated by authorized representatives of the Head, Secretary, or Executive Director. They shall be in charge of evaluating suggestions, documenting the results, and referring them to each division or office director, so that the necessary changes may be implemented to improve the quality of the services offered.
