Every [developer] or constructor who must obtain or renew a permanent or provisional license to engage in the home urbanization or construction business in Puerto Rico shall file a petition under oath on the blank forms, which to such effects the Construction Officer shall adopt and provide, in which he shall inform, in addition to what is required through regulation, the following:
(a) Name and address of the petitioner; the address of all the member representatives of the firm, if a partnership; name and address of all directors, officers and of the resident agent, if a corporation.
(b) Certified copy of the corporate charter of a partnership or of the certificate of incorporation as it was on the date of the filing of the petition.
(c) Urbanization or construction projects which are being carried out or which he intends to carry out, should this last fact be known.
(d) Evidence that the petitioner has the necessary technical knowledge in the projects which he is carrying out or intends to carry out, as well as the number, kind and type of projects constructed prior to the date of his petition and value thereof.
(e) Capital and available credit.
(f) Information proving that the petitioner has an organization capable of providing services subsequent to the delivery of the urbanization or construction.
In the case of provisional licenses the report on urbanization or construction projects carried out in Puerto Rico shall not be required.
