§ 55. Commonwealth homes for children—Admission of children

Admission of a child to the Commonwealth homes may be applied for by the parents, by the guardians, by any other person or entity interested in the child’s welfare, or by the Division itself through its child welfare agencies. The application shall be addressed to the Division of Public Welfare, Department of the Family, and shall contain the child’s name, the applicant’s name, and his/her relation to the child, if any, the name of the town the child comes from, the certificate of the keeper of the vital statistics registry relative to birth registration inclusive, and the reasons for applying for admission.
