(a) All protection orders must specifically indicate the orders issued by the court, the remedies ordered, and the period the order is to be in effect.
(b) All protection orders must state the date and time they were issued, and specifically notify the respondent that any violation thereof shall constitute contempt of court, which could result in imprisonment, a fine, or both penalties.
(c) Any ex parte protection order must include the date and time it was issued, and must state the date, time, and place where the hearing shall be held for the purpose of extension or annulment thereof, and the reasons that made it necessary to issue the ex parte order.
(d) All protection orders issued by a court shall be listed in a form substantially similar in content to the one included in this chapter as a guideline.
