Section 5-10-8-14.9 - Coverage of methadone(a) This section applies to an employee health plan that is established, entered into, amended, or renewed after June 30, 2015.(b) As used in this section, "state employee health plan" means one (1) of the following: (1) A self-insurance program established under section 7(b) of this chapter to provide group health coverage.(2) A contract with a prepaid health care delivery plan that is entered into or renewed under section 7(c) of this chapter.(c) A state employee health plan may provide coverage for methadone if the drug is prescribed for the treatment of pain or pain management as follows: (1) If the daily dosage is not more than sixty (60) milligrams.(2) If the daily dosage is more than sixty (60) milligrams, only if: (A) prior authorization is obtained; and(B) a determination of medical necessity has been shown by the provider.Amended by P.L. 149-2016, SEC. 18, eff. 3/23/2016.Added by P.L. 209-2015, SEC. 1, eff. 7/1/2015.
