Section 31-19-18-3 - Voluntary transmission of medical information; storage; affirmation(a) Any person may voluntarily transmit medical information to the state registrar for inclusion with the medical history.(b) The state registrar shall store all information received under this section in a manner that makes the information readily recoverable.(c) Any transmission of voluntary information must include an affirmation by the person that: (1) the information is true; or(2) the person believes the information is true.Pre-1997 Recodification Citation: 31-3-4-13.
