The name and address of and other personally identifiable information concerning a person whose death or serious injury is reported to the executive director of the nonprofit entity designated by the Governor in accordance with section 46a-10b to serve as the Connecticut protection and advocacy system, as required by the Developmental Disabilities Assistance and Bill of Rights Act of 2000, 42 USC 15041 et seq., as amended from time to time, and any regulations promulgated thereunder, and as required by the Protection and Advocacy for Individuals with Mental Illness Act, 42 USC 10801 et seq., as amended from time to time, and any regulations promulgated thereunder, pursuant to section 46a-153, the name and address of and other personally identifiable information concerning any person who provides information obtained by such nonprofit entity in the course of an investigation of any such report, and all confidential records obtained by such nonprofit entity in the course of any such investigation shall be confidential and shall not be subject to disclosure under the Freedom of Information Act, as defined in section 1-200. Nothing in this section shall be construed to prohibit such nonprofit entity from disclosing personally identifiable or confidential information with the consent of a person authorized by law to consent to the release of such information or from issuing reports to the public or providing information to policy-making bodies that contain statistical data, analysis or case studies, provided such nonprofit entity shall not disclose the identity of any person with a disability or any means of discovering such identity.
