Except, as otherwise provided, this chapter applies to all health benefit plans issued, made effective, delivered or renewed to any eligible small business after April first, nineteen hundred and ninety-two, whether issued directly by a carrier, or through an intermediary. Nothing in this chapter shall be construed to require a carrier which does not issue health benefit plans subject to the provisions of this chapter to issue health benefit plans subject to the provisions of this chapter.
