Section 3.2-3918 - Statement to be filed by registrantA. The registrant shall file a statement with the Commissioner including: 1. The name and address of the registrant and the name and address of the person whose name will appear on the label, if other than the registrant;2. The name of the pesticide;3. A complete copy of the labeling accompanying the pesticide and a statement of all claims made and to be made for it including directions for use;4. If requested, a full description of the tests made and the results thereof upon which the claims are based; and5. Other information requested by the Board such as product efficacy, all known health and environmental impacts, and known incidents of human or wildlife illnesses.B.
