Section 5-404 - Application for assistance(a) An application for assistance under this part shall be made:(1) to the local department of the county where the applicant resides; and(2) in the form and manner that the Administration requires.(b) Whenever a local department receives an application for assistance under this part, the local department shall make a record of:(1) the circumstances of the applicant;(2) the facts supporting the application; and(3) any other information that the Administration requires by regulation.
