1.a. The secretary may prescribe and furnish on request forms, including but not limited to the following: (1) An application for a certificate of existence.(2) A foreign cooperative's application for a certificate of authority to transact business in this state.(3) A foreign cooperative's application for a certificate of withdrawal.b. If the secretary so requires, use of these listed forms prescribed by the secretary is mandatory.2.
