Introductory Statement
1. The following forms are intended for illustration only, but they are expressly declared by Rule 46 to be sufficient.
2. The notice of appeal, Form 1, is to be filed in the court or other tribunal appealed from. Its caption is thus similar to the captions illustrated in the Appendix of Forms to the Vermont Rules of Civil Procedure. Other documents should be entitled or captioned for the Supreme Court as provided in Forms 2 and 3, or in the appropriate court as provided in Rule 4. See Rules 32, 24.
3. The names and addresses of counsel for the party filing the document must appear on the cover or cover sheet of a brief or the printed case. Rule 32(a). See Form 2. Each motion or similar paper must be signed individually by at least one attorney of record, whose address shall also appear. Rule 32(b). See Form 3.
4. The forms are for convenience given the same numbers as those in the Appendix to the Federal Rules of Appellate Procedure, except that federal forms inappropriate to state practice have been replaced by state forms.
