Rule 205.2(b) - Cover SheetAll applications requiring action by the Court, and any responses thereto, shall be accompanied by a cover sheet as the first page of the application. The cover sheet shall be substantially in the form as set forth after this Rule. The cover sheet shall include the following information:
(1) Identification of the Court;(2) The complete caption of the case;(3) Identifying case number;(4) If a matter in the case has been considered by a judge, the name of the judge shall appear immediately under the case number. If the case has been assigned to a judge, the judge's name shall appear under the case number and under the names of any judges who previously considered any matter in the case;(5) The identifying case numbers of any related cases;(7) The names, addresses and telephone numbers of primary counsel for all represented parties with identification of the party represented by counsel and all pro se parties;(8) The title of the pleading being submitted; and(9) The name of the party submitting the pleading.Click here to view image
