In a proceeding for the appointment of a guardian for an adult respondent, the notice required under ORS 125.060 shall include the following language or its equivalent:
"Free legal services for people at least sixty (60) years of age who are subject to guardianship proceedings may be obtained by calling Oregon Legal Services' Senior Law Program at 1-888-2454091. Help finding a lawyer and low-cost one-time legal consultation may be obtained by calling the Lawyer Referral Service of the Oregon State Bar at 1-800-452-7636."
