Rule 6-126 - Waiver or Consent
(a) Generally. A person may waive the right to any notice or may consent to any matter. The waiver or consent shall set forth the specific matter that is the subject of the waiver or consent, shall be signed, and shall be filed with the register and served on the personal representative. A person may revoke a waiver or consent at any time by filing a revocation with the register and serving it on the personal representative. The revocation shall have prospective effect only.
(b) Form of Waiver. A waiver of notice shall be filed with the register in the following form:

The May 9, 2000, order, in section (b), in the forms, deleted the prefix "19 in the year.
