In cases where applicable, a questionnaire in a form approved by the Court shall be submitted to the Jury and the answers thereto shall be made available to the parties prior to the commencement of Voir Dire examination. All questionnaires shall be returned to the Jury Commission Staff immediately at the conclusion of the selection process. Neither counsel nor the litigants shall mark or write on the questionnaire forms. No contact shall be made with Jurors before, during or after a trial without leave of the Trial or Chief Judge.
