a. Description of Case. Plaintiff or petitioner shall describe in the caption of the complaint or original petition filed with the Court the nature of the domestic relations action or proceedings as follows: Dissolution, Paternity, Maternity, Reciprocal, Foreign Domestic Relations Decree, Legal Separation, Annulment, Custody, Child Support, Parenting Time or other.b. Adoption and Severance Cases. In all adoption and severance matters filed before the Court, the caption for such case shall not include the full name of the child(ren) who is(are) the subject of the adoption proceedings. Instead, the child's initials (first name, middle name, last name) and date of birth shall be used for purposes of each child's identification in the caption.L. R. Prac. Sup. Ct. 18.1
