Rule 38 - Reports to Bureau of Vital Statistics-Superior Courts(a)Divorce - Annulment - Adoption. Before judgment or decree is entered in any action for divorce or annulment or proceeding for adoption, the court shall require the parties or their counsel to submit such personal particulars and other information necessary to enable the clerk to prepare a record of such divorce, annulment or adoption in accordance with law and the regulations and instructions of the Bureau of Vital Statistics. Every such record shall be prepared by the clerk and filed in the manner and within the time prescribed by law and the regulations and instructions of the Bureau of Vital Statistics.(b)Change of Name - Delayed Birth Certificate - Legitimation. In the following actions and proceedings, the court shall file with the Bureau of Vital Statistics such reports, information and copies of judgments and orders as may be required and in the manner provided by law and the regulations and instructions of the Bureau: (1) A proceeding for change of name;(2) A proceeding to establish a public record of the time and place of birth and parentage;(3) An action or proceeding for legitimation. Before entering any order or judgment in any of the above mentioned actions or proceedings, the court shall require the parties or their counsel to submit such information as may be necessary to enable the court to comply with this rule.
