The forms contained in "Appendix I. Forms; Examples" are provided as general examples and should be modified to comport with the particular circumstances of each case; they have not been approved or adopted by the Alabama Supreme Court as officially approved forms.

Comment to Rule 50 as Amended Effective May 8, 2001.
The forms appearing in Appendix I: Forms; Examples have not been approved by the Supreme Court as "officially approved forms." See Rule 50. The Unified Judicial System's Forms Committee may from time to time draft additional forms, some of them to replace forms included in Appendix I. These new forms are distributed to circuit court clerks and district court clerks for inclusion in the "Court Forms Manual." Many of these forms, as periodically updated, may be found at the Administrative Office of Courts' Web site: www.alacourt.org/, under the heading "UJS Court Forms."
