Section 4-4 - Issuance of Licenses and Certificates

The Board shall issue a wall document to each successful applicant bearing the full name of the holder, discipline designation, license or certification serial number, date of issuance, expiration date, and appropriate seal. Upon request, a second wall document will be issued if the individual has a need to display the document in more than one (1) place of business.
