Section 4-3 - Provider Invoice

(a) The Division shall provide a written invoice to the provider and parent for signature. The invoice shall contain at least the following information:

• (i) Provider name, address, and phone number;

• (ii) Rate of payment;

• (iii) Rate of parent co-payment; and

• (iv) Duration.
