Section 4-11 - Forms and Audit Verification

All renewal applications will require the licensee to provide the total number of CPC credits that he/she is claiming for the two (2) year period prior to license renewal. If selected for a random audit, the licensee must supply sufficient detail to permit verification as described in Section 7. Current Board members will be audited each time his/her license is renewed.
