Payment of fees must be in United States dollars, and may be in cash, check, and money order, credit card or any payment mechanism approved by the board. If a check is returned for insufficient funds, repayment, including payment of a returned check charge, shall be in cash or by money order or certified check.
(a) Combined alphabetical/numerical roster of licensees - $50
(b) Address Labels - $100
(c) Copy fee per page - $ 0.25
(d) Replacement certificates - $10
(e) Returned check charge - $25
