The application by a peace officer, detention officer, correctional officer, or dispatcher for certification shall be made by completing the Employment Form provided by the Commission. (This form shall be submitted to the Commission within fifteen (15) days of appointment.) W.S. 9-1-704(a) and W.S. W.S. 9-1-710(a)
