Section 24-1-9 - Temporary Permit to Practice Osteopathic Medicine and Surgery; Application Required9.1. An applicant for a temporary permit to practice osteopathic medicine and surgery in West Virginia shall submit an application on a form prescribed and provided by the Board. The application is available on the Board's website. The form shall be completed and submitted at least 30 days in advance of the date on which the expected practice will begin, together with the following documents: 9.1.1. Evidence of graduation from a medical school accredited by the AOA;9.1.2. A photograph taken within 60 days which resembles the applicant;9.1.3. A letter from a physician fully licensed to practice osteopathic medicine and surgery in West Virginia who has agreed to supervise the applicant, if considered necessary by the Board;9.1.4. A non-refundable fee in an amount established by the West Virginia Board of Osteopathic Medicine rule Fees for Services Rendered By the Board of Osteopathic Medicine, 24 CSR 5; and9.1.5. Evidence of successful completion of a minimum of one year of post-doctoral clinical training in a program approved by the American Osteopathic Association or the Accreditation Council for Graduate Medical Education.9.2. The application, together with all photocopied documents submitted with it, become the property of the Board and shall not be returned.9.3. An applicant for a temporary permit shall arrange for a personal interview with a member of the Board prior to the meeting at which his or her application is to be considered. The Board may require that an applicant be present at the meeting during which his or her application will be reviewed. The purpose of that interview or required attendance at a Board meeting is to clarify any information contained in the application. The Board may require production of original documents at the interview or required attendance at a Board meeting.
