Section 126-85-64 - Release of Information64.1. Information obtained from families on the free/reduced lunch forms is to be used for determining student's eligibility for meal benefits. 64.2. This information may also be used to determine eligibility for free textbooks, workbooks and school supplies, and to provide student information to access Medicaid and CHIP benefits. The release of this information may only be authorized by the parent or guardian in Section VII of the application. 64.3. Information may also be released to specific educational officials and other agencies sponsoring USDA child nutrition programs so a child may receive additional benefits. W. Va. Code R. § 126-85-64
