Section 388-829R-150 - What are the requirements for record entries?(1) The overnight planned respite services provider must make entries at the time of or immediately following the event and maintain both the original and corrected entries when an error in the record is made.(2) Written entries must be legible, in ink, and signed and dated.(3)Electronic record entries must identify the date of the entry and the person who made the entry by his or her unique user ID.Wash.
