The following is a design for a typical curb ramp:

Texturing of center ramp recommended, with metal grid placed in wet concrete and then removed to leave ridged surface pattern. Pattern not to exceed 1/2" width.

Advance warning strip (shown dotted), necessary when ramp is in direct line of main pedestrian path.
