Section R698-12-4 - Applications(1) A child applying for scholarship funding shall submit the application to the department: (a) on a form approved by the department;(b) with supporting documentation that reflects the educational-related expenses for which the funding will be used; and(c) between January 1 and March 31 of each year, for up to four years.(2) Applications received by the department from an applicant after funding has been granted to the applicant for four years shall be denied.Utah Admin.
