Section R436-9-3 - Penalty for Failing to Complete a Required Monthly Record and Reporting of BirthsA facility, facility representative, or midwife violates this rule and is subject to the penalties provided in Section 26B-1-224, including both administrative and civil penalties if they:
(1) fail to keep a monthly record of births occurring in the hospital, birthing center, or delivered by a midwife;(2) fail to submit the monthly record of births to the state registrar; or(3) fail to keep files of necessary information to complete birth certificates for deliveries occurring in the hospital, birthing center, or by a midwife.Utah Admin.
