Section R436-3-4 - Amend a Record(1) All amendments to vital records shall be requested by submitting a notarized affidavit asserting that the error exists signed by the person so asserting and one other credible person having knowledge of the facts. The affidavit will set forth: (a) information to identify the record;(b) the items to be amended;(c) the incorrect information as it appears; and(d) the correct information as it should appear.Utah Admin.
