Section R414-22-8 - Notice of Sanction(1) If a provider is sanctioned for a period exceeding 15 days, the department may notify the applicable professional society, board of registration or licensor, and federal or state agencies.(2) The department shall include in the notice: (a) the findings made; and(b) the sanctions imposed.(3) The department shall timely notify any appropriate Medicaid member of the provider's suspension or termination from the Medicaid program.Utah Admin.
