Section R414-10A-5 - Service Coverage(1) Transplantation services are covered by the Utah Medicaid program only when requirements in this rule are met. (2) Multi-organ transplantation services may be provided only when the requirements for each individual transplant are met. (3) Repeat transplantations of the same tissues or organs may be covered only under Departmental review and approval based on requirements in this rule. (4) The following transplants are covered when requirements in this rule are met: (a) Cornea, heart, lung, kidney, liver, pancreas, intestine, bone marrow, hematopoietic stem cell. (b) Some combinations of the above may also qualify. (5) Emergency transplantations may be covered if all requirements are met. Utah Admin. Code R414-10A-5
