Section R398-5-4 - Birth Defects Reporting by LaboratoriesEach laboratory operating in the state that identifies a human chromosomal or genetic abnormality or other evidence of a birth defect shall report the following on a calendar quarterly basis to the department within 40 days of the end of the preceding calendar quarter:
(1) if live born, the child's last name;(2) if live born, the child's first name;(3) the child's date of birth;(4) the mother's last name;(5) the mother's first name;(6) the mother's date of birth;(7) the date the laboratory accepted the sample;(10) the mother's state of residency at delivery.Utah Admin.
