Section R398-5-4 - Birth Defects Reporting by Laboratories

Each laboratory operating in the state that identifies a human chromosomal or genetic abnormality or other evidence of a birth defect shall report the following on a calendar quarterly basis to the department within 40 days of the end of the preceding calendar quarter:

(1) if live born, the child's last name;
(2) if live born, the child's first name;
(3) the child's date of birth;
(4) the mother's last name;
(5) the mother's first name;
(6) the mother's date of birth;
(7) the date the laboratory accepted the sample;
(8) the test conducted;
(9) the test result; and
(10) the mother's state of residency at delivery.
