Section R154-1-4 - Means to Deliver CFS Filings; Forms(1) A CFS filing shall be filed electronically through the central filing system or on a paper-based CFS form.(2) Paper-based CFS forms are approved by the Division from time to time, and a list of CFS forms approved by the Division will be made available upon request. (3) A CFS filing is effective for a period of five years from the date of filing, except that a continuation CFS filing will extend the effective period for a CFS filing for an additional five years.Utah Admin.
