Section R70-360-3 - License Requirements(1) A license shall be issued to an individual applicant and is not transferable.(2) Licenses shall expire on December 31 of each year.(3) To get a license, an applicant shall demonstrate testing proficiency by successfully completing a series of split samples provided by the department.(4) After an applicant meets the criteria for licensing, the applicant shall submit an application form with the license fee to the department. The license fee shall be determined by the department pursuant to Section 4-2-103.(5) The department shall issue a license to the applicant when an application form and fee are received.Utah Admin.
