Section R66-5-19 - Pharmacy License and Renewal(1) Each cannabis pharmacy license shall expire one calendar year from the date of licensure.(2)(a) A medical cannabis pharmacy shall submit a notice of intent to renew to the department within 30 days of receiving a notice of expiration from the department.(b) If the intent to renew is not submitted to the department, the licensee may not continue to operate.(3) If the licensing fee is not paid by the expiration date, the licensee may not continue to operate.(4) The board may take into consideration significant violations issued in determining license renewals.Utah Admin.
