Section 355.540 - Reclassification of High- or Moderate-Risk Suicidal YouthReclassification of a resident designated as high or moderate risk for suicidal behavior to a lower risk level shall only be determined by the facility administrator with the recommendation of a qualified mental health professional, a mental health provider, or a licensed physician.
(1) Prior to recommending reclassification, a qualified mental health professional, mental health provider, or a licensed physician shall conduct a review of the resident's current suicide risk and issue a written recommendation which addresses the following: (A) the need to reclassify the resident's suicide risk level;(B) the need for intervention strategies and/or services during the resident's period of confinement within the facility; and(C) the need for additional assessment(s), screening(s) or evaluation(s).(2) The written recommendation of the qualified mental health professional, mental health provider, or licensed physician shall be maintained in the resident's record.(3) The facility administrator or designee shall review the written recommendation of the qualified mental health professional, mental health provider, or licensed physician prior to reclassifying a resident as no longer being at high risk for suicidal behavior.(4) Only the facility administrator or designee shall authorize the reclassification of a resident classified as high risk for suicidal behavior under this section.37 Tex.
