Section 355.236 - Data CollectionThe facility administrator or chief administrative officer shall maintain and report to TJJD electronically, or in the format requested, accurate statistics in the following areas:
(1) total number of grievances;(2) total number of personal restraint incidents;(3) total number of mechanical restraint incidents;(4) total number of disciplinary separations; and(5) total number of staff injuries resulting from interaction with residents.37 Tex. Admin. Code § 355.236
