Section 6.201 - Captive Insurance Company Certificate of Authority Required(a) A person seeking a certificate of authority to act as a captive insurance company must submit an application and payment of the application fee in a format acceptable to the department and provide the information required in § 6.202 of this title (relating to Captive Insurance Company Certificate of Authority Application Contents and Process).(b) The application fee is $1,500.(c) The department may provide forms that an applicant can use for submitting its application.28 Tex.
