The Department adopts and incorporates by reference standard administration forms for use in the administrative regulation of premium finance companies. Applicants and licensed premium finance companies shall utilize the forms in preparing applications, statements, notices of required information, and other submissions required under the Insurance Code, Chapter 24, and this chapter. These forms are published by the Department and may be obtained from the Premium Finance Licensing Unit, Mail Code 107-5A, Texas Department of Insurance, 333 Guadalupe, P.O. Box 149104, Austin, Texas 78714-9104. These forms are identified as follows.
(1) FORM PF1--Premium Finance Company License Application (for NEW license).(2) FORM PF1A--Premium Finance Supplemental Application (for additional location, relocation, name change and ownership change).
