Section 558.300 - Medication Administration(a) This section applies only to clients to whom agency staff administer medications.(b) An agency must adopt and enforce a written policy for maintaining a current medication list and a current medication administration record. (1) A client's practitioner must order administration of medication.(2) An agency may incorporate a current medication list and medication administration record into one document. (A) An agency must use the medication list to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contraindications.(B) An agency must document in the medication administration record or clinical notes any medication that is not administered and the reason it was not administered.(3) An individual delivering care must report any adverse reaction to a supervisor and document this in the client's record on the day of occurrence. If the adverse reaction occurs after regular business hours, the individual delivering care must report the adverse reaction as soon as it is disclosed.26 Tex. Admin. Code § 558.300
