Section 343.50 - Request for Reinstatement of License
(a) At the expiration of 180 days from the date of revocation, the Investigation Committee may consider a request for reinstatement by the former licensee.
(b) The request for reinstatement must be submitted to the agency in writing and should include a short and plain statement of the reasons why the requestor believes the license should be reinstated.
(c) Upon denial of any request for reinstatement, the board may not consider a subsequent request until the expiration of one year from the date of denial of the prior request.
(d) In taking action to revoke a license, the board may, in its discretion, specify the terms and conditions upon which reinstatement shall be considered.
