Section 1200-06-01-.11 - RETIREMENT AND REACTIVATION(1) A person who holds a current license and does not intend to practice as a Medical Laboratory Professional in Tennessee may apply to convert an active license to retired status. An individual who holds a retired license will not be required to pay a renewal fee.(2) A person who holds an active license may apply for retired status in the following manner: (a) Obtain from the Board's administrative office or via the Internet an affidavit of retirement form; and(b) Complete and submit the affidavit affirming that, while in retired status, the licensee will not practice or in any way indicate or imply that s/he holds an active Tennessee license.(3) License holders whose licenses have been retired may reactivate their licenses in the following manner:(a) Submit a written request for licensure reactivation to the Board's administrative office;(b) Pay the current licensure renewal fees and State regulatory fee as provided in Rule 1200-06-01-.06. If retirement reactivation is requested prior to the expiration of one year from the date of retirement the Board will require payment of the late renewal fee and all past due licensure renewal fees as prescribed in Rule 1200-06-01-.06.(c) Submit evidence of successful completion of the continuing education requirements pursuant to Rule 1200-06-01-.12.(4) Licensure reactivation applications shall be treated as licensure applications and review and decisions shall be governed by Rule 1200-06-01-.07.Tenn. Comp. R. & Regs. 1200-06-01-.11
