Section 1200-06-01-.09 - RENEWAL OF LICENSE(1) Renewal Application(a) Pursuant to the Division of Health Related Board's biennial birthdate renewal system, the due date for renewal, which is indicated on the license, is the last day of the month in which a licensee's birthdate falls. The licensee is personally responsible for meeting all requirements for renewal prior to the expiration date to legally continue in practice.(b) Methods of Renewal - Licensees may accomplish renewal by one (1) of the following methods:2. Paper Renewals - If a licensee does not renew via the Internet, s/he shall submit the following to the Division of Health Related Boards on or before the expiration date: (i) A completed renewal application form; and(ii) The renewal and State regulatory fees as provided in Rule 1200-06-01-.06.(c) Anyone submitting a renewal form, electronically or otherwise, which is found to be fraudulent or untrue may be subject to disciplinary action.(d) Licensees who fail to comply with the renewal rules or notification received by them concerning failure to timely renew shall have their licenses processed pursuant to Rule 1200-10-01-.10.(e) For persons licensed under national certification requirements as provided in Rules 1200-06-01-.20, 1200-06-01-.22, or 1200-06-01-.24, proof of current national certification shall be submitted, if requested by the Board, with the renewal application.(2) Reinstatement of an Expired License(a) Licenses that have expired may be reinstated upon meeting the following conditions:1. Payment of all past due renewal and State regulatory fees;2. Payment of the late renewal fee provided in Rule 1200-06-01-.06;3. Submission of evidence of successful completion of the continuing education requirements pursuant to Rule 1200-06-01-.12; and4. Completion of a reinstatement form.(b) Renewal issuance decisions pursuant to this rule may be made administratively or upon review by any Board member or the Board's designee.Tenn. Comp.
